Abstract


This study was carried out on the impact of poor communication and team collaboration as a tool for effective health care service delivery in federal teaching hospital Ido-Ekiti. The survey design was adopted and the simple random sampling techniques were employed in this study. The population size comprise of staff of federal teaching hospital Ido-Ekiti. In determining the sample size, the researcher purposefully selected 53 respondents and 50 were validated. Self-constructed and validated questionnaire was used for data collection. The collected and validated questionnaires were analyzed using frequency tables. While the hypotheses were tested using chi-square statistical tool. The result of the findings reveals that poor communication affects effective health care service delivery in federal teaching hospital Ido-Ekiti.The study also revealed that team collaboration promotes effectiveness in delivering healthcare services. The findings of the study further revealed that effective communication will promote team collaboration, hence facilitating excellent healthcare service delivery in federal teaching hospital Ido-Ekiti. Therefore, it is recommended that team building activities should be designed, planned, executed and monitored in such a way that it will enhance open communication, collaboration and flexibility in order to create effective cohesion and synergy to enhance team effectiveness and performance among healthcare workers. And team’s esprit de corps is a situation in which a group of people jointly depend on one another. This should be strengthened by making sure that people of like minds are put together in a team so that the hospitals can enjoy the full potential of such teams. To mention but a few.

Chapter One


Introduction

1.1 Background of the Study

Webster’s Dictionary defines communication as “the imparting or interchange of thoughts, opinions, or information by speech, writing, or signs.” It is important to consider that communication is not just verbal in form. One study states that 93 percent of communication is more affected by body language, attitude, and tone, leaving only 7 percent of the meaning and intent based on the actual words said. Whereas the spoken words contain the crucial content, their meaning can be influenced by the style of delivery, which includes the way speakers stand, speak, and look at a person. However, critical information is often transmitted via handwritten notes, e-mails, or text messages, which can lead to serious consequences if there is miscommunication (Guzzo & Dickenson 1996).

Collaboration in health care is defined as health care professionals assuming complementary roles and cooperatively working together, sharing responsibility for problem-solving and making decisions to formulate and carry out plans for patient care. Collaboration between physicians, nurses, and other health care professionals increases team members’ awareness of each others’ type of knowledge and skills, leading to continued improvement in decision making.

Effective teams are characterized by trust, respect, and collaboration. Deming8 is one of the greatest proponents of team collaboration. team collaboration, he believes, is endemic to a system in which all employees are working for the good of a goal, who have a common aim, and who work together to achieve that aim. When considering a team collaboration model in health care, an interdisciplinary approach should be applied. Unlike a multidisciplinary approach, in which each team member is responsible only for the activities related to his or her own discipline and formulates separate goals for the patient, an interdisciplinary approach coalesces a joint effort on behalf of the patient with a common goal from all disciplines involved in the care plan. The pooling of specialized services leads to integrated interventions. The plan of care takes into account the multiple assessments and treatment regimens, and it packages these services to create an individualized care program that best addresses the needs of the patient. The patient finds that communication is easier with the cohesive team, rather than with numerous professionals who do not know what others are doing to mange the patient (Obansa & Osrimisan 2013).

It is important to point out that fostering a team collaboration environment may have hurdles to overcome: additional time; perceived loss of autonomy; lack of confidence or trust in decisions of others; clashing perceptions; territorialism; and lack of awareness of one provider of the education, knowledge, and skills held by colleagues from other disciplines and professions. However, most of these hurdles can be overcome with an open attitude and feelings of mutual respect and trust. A study determined that improved team collaboration and communication are described by health care workers as among the most important factors in improving clinical effectiveness and job satisfaction.


1.2 Statement of the Problem

Quality health is a fundamental right of all Nigerian citizens. While health care (PHC) centers are relatively uniformly distributed throughout local government areas (LGAs) in Nigeria, the rural people tend to underuse the basic health services and this is mainly due to poor communication and team collaboration of the staff(Obansa & Osrimisan 2013). The responsibility for perpetuating poor health delivery system should be held by policy makers and specifically the lack of communication and team work. Responsible health personnel can build a new social order, based on greater equity and human dignity, in which health for all by the year 2015, including that of rural populations, will no more be a dream but a reality (Obansa & Osrimisan 2013). Capacity building and empowerment of communities through orientation, mobilization and community organization as regards training, information sharing and continuous dialogue, could further enhance the utilization of health services by the populations.


1.3 Objectives of the Study

The main aim of this study is to examine the impact of poor communication and team collaboration as a tool for effective health care service delivery in federal teaching hospital Ido-Ekiti.
Other specific aims of this study are:

  1. To determine the extent at poor communication affect team collaboration in federal teaching hospital Ido-Ekiti.
  2. To determine whether poor communication affects effective health care service delivery in federal teaching hospital Ido-Ekiti.
  3. To ascertain whether team collaboration promotes effectiveness in delivering healthcare services.
  4. To determine whether effective communication will promote team collaboration, hence facilitating excellent healthcare service delivery in federal teaching hospital Ido-Ekiti.

1.4 Research Questions

The following research questions will be answered in this study:

  1. To what extent does poor communication affect team collaboration in federal teaching hospital Ido-Ekiti?
  2. Does poor communication affect effective healthcare service delivery in federal teaching hospital Ido-Ekiti?
  3. Does team collaboration promote effectiveness in delivering healthcare services?
  4. Will effective communication promote team collaboration, hence facilitating excellent healthcare service delivery in federal teaching hospital Ido-Ekiti?

1.5 Research Hypotheses

The following null hypotheses will validate this study:

  • H0: Effective communication will not promote team collaboration, hence facilitating excellent healthcare service delivery in federal teaching hospital Ido-Ekiti.
  • Ha: Effective communication will promote team collaboration, hence facilitating excellent healthcare service delivery in federal teaching hospital Ido-Ekiti.

1.6 Significance of the Study

The findings of this study will aid the bodies in charge of health care service delivery as it will highlight the importance of communication and team collaboration. It will also assist service providers at the facility level in developing strategies and action plans to encourage increased use of the services available. These efforts will help to start the process of enhancing access to health care services, which will eventually improve the overall health of everyone in Ido.

This study will contribute to the current literature in this field and will also serve as a resource for academics, researchers, and students who may want to do future research on this topic.


1.7 Scope of the Study

This study focuses on the impact of poor communication and team collaboration as a tool for effective health care service delivery in federal teaching Ido. Specifically, this study focuses on determining the extent at poor communication affect team collaboration in federal teaching hospital Ido-Ekiti, determining whether poor communication affects effective health care service delivery in federal teaching hospital Ido-Ekiti, ascertaining whether team collaboration promotes effectiveness in delivering healthcare services, and determine whether effective communication will promote team collaboration, hence facilitating excellent healthcare service delivery in federal teaching hospital Ido-Ekiti.


1.8 Limitations of the Study

In the course of carrying out this study, the researcher experienced some constraints, which included time constraints, financial constraints, language barriers, and the attitude of the respondents. However, the researcher were able to manage these just to ensure the success of this study.

Moreover, the case study method utilized in the study posed some challenges to the investigator including the possibility of biases and poor judgment of issues. However, the investigator relied on respect for the general principles of procedures, justice, fairness, objectivity in observation and recording, and weighing of evidence to overcome the challenges.


1.9 Definition of Terms

Communication:

Communication is “an apparent answer to the painful divisions between self and other, private and public, and inner thought and outer world

Team Collaboration:

Team collaboration is a communication and project management approach that emphasizes team collaboration, innovative thinking and equal participation to achieve objectives.


1.10 Organization of the Study

The study is categorized into five chapters. The first chapter presents the background of the study, statement of the problem, objective of the study, research questions and hypothesis, the significance of the study, scope/limitations of the study, and definition of terms. The chapter two covers the review of literature with emphasis on conceptual framework, theoretical framework, and empirical review. Likewise, the chapter three which is the research methodology, specifically covers the research design, population of the study, sample size determination, sample size, abnd selection technique and procedure, research instrument and administration, method of data collection, method of data analysis, validity and reliability of the study, and ethical consideration. The second to last chapter being the chapter four presents the data presentation and analysis, while the last chapter(chapter five) contains the summary, conclusion and recommendation.

5.1 Introduction

This chapter summarizes the findings on the impact of poor communication and team collaboration as a tool for effective health care service delivery in federal teaching hospital Ido-Ekiti. The chapter consists of summary of the study, conclusions, and recommendations.


5.2 Summary of the Study

In this study, our focus was on the impact of poor communication and team collaboration as a tool for effective health care service delivery in federal teaching hospital Ido-Ekiti. The study is was specifically carried out to determine the extent at poor communication affect team collaboration in federal teaching hospital Ido-Ekiti, determine whether poor communication affects effective health care service delivery in federal teaching hospital Ido-Ekiti, ascertain whether team collaboration promotes effectiveness in delivering healthcare services, and determine whether effective communication will promote team collaboration, hence facilitating excellent healthcare service delivery in federal teaching hospital Ido-Ekiti.

The study adopted the survey research design and randomly enrolled participants in the study. A total of 50 responses were validated from the enrolled participants where all respondent were staff of federal teaching hospital Ido-Ekiti.


5.3 Conclusions

Based on the findings of this study, the researcher concluded that;

  1. The extent at poor communication affect team collaboration in federal teaching hospital Ido-Ekiti is high.
  2. Poor communication affects effective health care service delivery in federal teaching hospital Ido-Ekiti.
  3. Team collaboration promotes effectiveness in delivering healthcare services.
  4. Effective communication will promote team collaboration, hence facilitating excellent healthcare service delivery in federal teaching hospital Ido-Ekiti.

5.4 Recommendation

Based on the responses obtained, the researcher proffers the following recommendations:

  1. Team building activities should be designed, planned, executed and monitored in such a way that it will enhance open communication, collaboration and flexibility in order to create effective cohesion and synergy to enhance team effectiveness and performance among healthcare workers.
  2. Team’s esprit de corps is a situation in which a group of people jointly depend on one another. This should be strengthened by making sure that people of like minds are put together in a team so that the hospitals can enjoy the full potential of such teams.
  3. Hospitals should develop effective approach to high performing teams by allowing open communication structure and empowerment evaluation to increase team effectiveness and performance. The selection of team members should be based on individual ability, technical skills and competencies in order to stimulate team member’s interest to task meant to be accomplished and in the fulfillment of the team’s role.
